Abstract

All’inizio del 2007 ho ricevuto da un’amica di famiglia la richiesta di interessarmi al caso della propria figlia, che al tempo compiva 18 mesi, emerso nelle fasi iniziali come un insistente problematica dermatologica caratterizzata da eritemi diffusi in varie zone del corpo, associati alla presenza di piccoli brufoli ed arrossamento generale.

La madre lamentava il fatto che nonostante l’utilizzo delle creme lenitive ad uso pediatrico, di cui tutti i genitori fanno uso in maniera più o meno discontinua, il fenomeno non accennava a scomparire ma anzi peggiorava piuttosto rapidamente, ed a nulla erano valsi i responsi e le terapie suggerite da vari medici che nel frattempo erano stati consultati. Inoltre vari pediatri e dermatologi si erano prodigati nell’emissione di diagnosi discordanti e talvolta contradditore fra di loro, lasciando la nostra amica alquanto perplessa relativamente alla via da seguire ed in profonda apprensione per l’evoluzione negativa del disturbo della piccola.

Ho accettato quindi di occuparmi del caso, ed ho deciso in seguito di approfondire le problematiche relative alla Dermatite Atopica (DA) e di presentarle in questo lavoro. Ho scoperto che la crescente diffusione delle sintomatologie atopiche dell’infanzia e dell’adolescenza è ampiamente documentata sia in Italia che in tutti i principali paesi industrializzati, ma nonostante questo la complessita e la multifattoriaità del disturbo rappresenta tuttora una sfida terapeutica.

La tesi si situa in un ambito di indagine scientifico orientato verso il campo delle terapie naturali. Si potrebbe definirlo come un lavoro di interfaccia, volto ad integrare i limiti dell’approccio medico convenzionale con il valore rappresentato dalle terapie naturali, e a colmare una certa lacuna relativa a come le terapie naturali basate su evidenze scientifiche possano essere coerentemente utilizzate.

Le evidenze scientifiche relative ai processi immunologici e infiammatori coinvolti nell’insorgenza, nello sviluppo e nell’evoluzione della DA sono abbondanti e consolidate. Il riferimento a tali evidenze assume parte preponderante in questo lavoro, in quanto le informazioni sulle modalità che il Naturopata può adottare nella DA sono frammentate e non sono così abbondanti e accessibili.

Questa tesi è divisa in due parti.

Nella prima parte si prendono in esame, dal punto di vista scientifico, le caratteristiche dei sistemi implicati nella problematica della DA: la cute con i suoi aspetti morfo-funzionali, il sistema immunitario con il suo complesso network citochinico, e la problematica DA con le sue caratteristiche e le diverse concause ed implicazioni.

Nella seconda parte si traccia un approccio per un intervento naturale al trattamento della DA. In questa parte si cerca di sottolineare la logica che dovrebbe presiedere a un protocollo terapeutico oggettivo, e si forniscono indicazioni sui alcuni rimedi da adottare in tale ottica, discutendo un approccio naturopatico alla DA ed un razionale terapeutico.

Questo lavoro vuole anche testimoniare la constatazione diretta di come l’impiego di terapie naturali possa sortire esiti incoraggianti, soprattutto se confrontati con gli esiti dei trattamenti convenzionali. Non ultimo, poichè evidenze recenti indicano un possibile ruolo delle terapie naturali nella riduzione dell’impatto della DA sul nascituro, si vuole notare come il Naturopata si trovi oggi nella condizione di poter adottare nuove ed evolute linee terapeutiche di intervento che, essendo applicabili in fase prenatale, costituiscono una vera azione di prevenzione primaria.
